CLOUD FOREST MARTIAL ARTS INSTITUTE — ONE-DAY VISITOR WAIVER, RELEASE, AND ASSUMPTION OF RISK I acknowledge that martial arts training carries inherent risks of physical injury. I agree to follow all safety instructions from instructors at Cloud Forest Martial Arts Institute and assume responsibility for my own well-being (or my child's well-being) during my visit. I release Cloud Forest Martial Arts Institute, Greensboro Martial Arts Academy LLC, its instructors, contractors, and staff from any liability for injuries sustained during my visit, except in cases of gross negligence. I confirm that any medical information I have provided is accurate, and I will inform staff of any conditions that may affect safe training. For a minor visitor, I confirm I am the parent or legal guardian and have authority to sign on the student's behalf. PHOTOGRAPHY AND MEDIA RELEASE: I understand that photographs, video, and audio may be recorded during classes, events, and other activities while I (or my child) am present at Cloud Forest Martial Arts Institute. I grant Cloud Forest Martial Arts Institute and Greensboro Martial Arts Academy LLC the irrevocable, perpetual, worldwide, royalty-free right to use, reproduce, edit, and publish this footage and my likeness in any medium, including for advertising, marketing, education, instructional and tutorial content, social media, and archival purposes. I waive any right of ownership, inspection, approval, or compensation regarding this material, and I release the Institute from any claim arising from its use. I may submit a written request to be excluded from future recording. COMMUNICATIONS CONSENT: I agree that, by signing, Cloud Forest Martial Arts Institute may add me to its newsletter and may contact me by email at the address I provided with school news, schedules, events, and promotions. I understand I may unsubscribe at any time using the link in any email. I understand that Cloud Forest is a Private Membership Organization. This waiver authorizes a single one-day guest visit only and does not grant club membership. After this one-day visit, I may not participate again without enrolling as a member, which requires the annual club fee and a tuition plan.

CLOUD FOREST MARTIAL ARTS INSTITUTE — CLUB RULES & CODE OF CONDUCT 1. Respect everyone. Treat instructors, fellow students, staff, and guests with courtesy at all times. Harassment, threats, profanity, and abusive behavior are not tolerated. 2. Follow instructor direction. Do as your instructors ask, and follow all safety rules and training etiquette on and off the floor. 3. Safety first. Train within your limits, never use a technique to harm a partner, and stop immediately when an instructor calls a halt. 4. Arrive ready. Come about 10 minutes early, wear clean comfortable clothing, remove your shoes before stepping onto the training floor, and keep nails trimmed and jewelry removed. 5. Good hygiene. Arrive clean and in clean clothing. Avoid heavy scents out of respect for sensitive training partners. 6. Train, do not instruct. Do not teach or correct other students unless an instructor asks you to. 7. Care for the space. Keep the training floor, changing areas, and equipment clean and orderly. Report any damage or hazard to staff. 8. No drugs or alcohol. Do not train under the influence of alcohol or any substance that impairs judgment or safety. 9. Supervise children. Parents and guardians are responsible for any children who are not in class. 10. A private club, for one day. Cloud Forest is a Private Membership Organization. A guest pass is valid for a single day only. To keep training afterward you must enroll as a member with the annual club fee and a tuition plan.
